Our team supports children & young people with Special Educational Needs or Disabilities (SEND).

Children are supported across age ranges of:

• Early Years 0-5 - establish early networks and guarantee a smooth move into childcare and school and help minimise difficulties in the future in line with the SEN Code of Practice.
• Primary 6-11 - establish early networks and guarantee a smooth move into primary education
• Secondary 12-16 - ensures that young people have a smooth transition into secondary school and receive the information they need to support them.
• Moving On 17-25 - ensure that the school and family begin to prepare for the young person's transition into adulthood/post-16 provision and that they have clear expectations about what educational options and services are available and make informed choices about the future.

The Early Help team is made up of the following professionals who as team work together to spot health problems or concerns early, so that we can provide early support. We work with other services to ensure each and every child and young person lives in a safe environment:

• Health Visitors are specialist community public health nurses (SCPHN) and registered nurses. They specialise in working with families with a child aged 0-5 years to identify health needs as early as possible and improve health and wellbeing by promoting health, preventing ill health and reducing inequalities.
• Community Nursery Nurses (CNN) provide a vital role within the child health team and use their expertise to provide packages of care for families previously assessed by a health visitor.
• Family Support Workers (FSW) take action as soon as possible to tackle issues with children and families before they can grow into something that is more difficult to overcome. They identify children and families that may be at risk of running into difficulties and providing timely and effective support.
• School Nurses are specialist community public health nurses (SCPHN) and registered nurses. They work with school-aged children and young people and their families to improve their health and wellbeing outcomes and reduce inequalities and vulnerabilities.

Within Children's Services we have dedicated teams working to support the families of Hartlepool on a formal and informal basis. We understand that most Families experience challenging times, and we are here to support them.

Fostering is often used to provide temporary care whilst parents receive help to sort out problems, or to help distressed and troubled children and young people through difficult periods in their lives.

Foster Carers have an allocated Social Worker from the Fostering Team known as a Supervising Social Worker. The role of your Social Worker is to supervise and support the Foster Carer when children/young people is in their your care visiting a minimum of every 4 weeks or more if required.

Youth Support Services offer a wide range of activities, advice and support from dedicated Youth Centres, the One Stop Shop and from various locations across the town via the Youth Service Mobile and Detached service.

The One Stop Shop offers young people a wide range of services including:

• Support into Education, Training or Employment
• Health and Relationship advice
• Support with money issues, benefits advice, student finance and budgeting
• Job search facilities including Curriculum Vitae support
• Support around housing issues
• Opportunities for personal and social development
• C Card, Chlamydia and pregnancy testing

These are where Family Support Workers, Family Hub Workers, Health Visitors, Midwifery Nurses and Community Nursery Nurses are based.

The Family Hubs work primarily with families with children from 0 to 19 (0-25 with SEND). They provide integrated childcare and education; health services; family support; and access to Job Centre Plus.

The Family Hubs are a one stop shop for free support for children and their families where skilled staff are on hand to give help, advice and guidance on such things as health advice, parenting and family support, play sessions, childcare, help to get a job or training, or simply a place to meet other parents and much more. The Family Hubs offer groups, parenting support, health visitor clinics, development reviews when your child is aged 9-12 months and 2-2.5 years old and advice and guidance on things like feeding your baby/child, toilet training, routines, safe sleeping, introducing solid foods and breastfeeding.

The Family Hubs run a wide range of groups for children on a weekly basis, such as sing and sign, free play time, baby weigh in, breastfeeding support, story time and so much more.

The hubs require a whole range of dedicated employees to ensure they are a fun, safe and caring place for the people of Hartlepool.
