Children are supported across age ranges of:
- Early Years 0-5 - establish early networks and guarantee a smooth move into childcare and school and help minimise difficulties in the future in line with the SEN Code of Practice.
- Primary 6-11 - establish early networks and guarantee a smooth move into primary education
- Secondary 12-16 - ensures that young people have a smooth transition into secondary school and receive the information they need to support them.
- Moving On 17-25 - ensure that the school and family begin to prepare for the young person's transition into adulthood/post-16 provision and that they have clear expectations about what educational options and services are available and make informed choices about the future.
