Hartlepool EHCP Reviews Process Feedback

We want to improve the quality of our EHCPs and your views play an important role in this. The results of this survey will be summarised and shared, along with any actions taken, in our annual “You said, We did” report.

You can fill in this survey if you are:

· A parent of a child or young person with an EHCP

· A young person over 16 with an EHCP

The results from this survey will be checked once every half term. If you have a question or have raised an issue which requires a response then please contact the SEND Team via SEN@hartlepool.gov.uk, or alternatively please ring 01429 523553 and your query will be redirected to the appropriate person. 

This is an anonymous survey and we are not asking you for any information identifying you or your child/young person. Please don't include any information in comments which will identify you or your child.
